On your first visit, you will receive an individualized detailed evaluation from your designated physical therapist. Depending on your insurance approval, you may or may not receive treatment on your first visit. For your convenience, we will email you the required forms upon your request and you can print and bring your forms to expedite your service. If the printer is not available, no problem. Please prepare to be 15 minutes early to fill out your paperwork. Please have a valid photo ID and insurance card ready.
You don’t have a prescription yet and are in severe pain? Call us immediately. In the state of California clients without a physician prescription can receive physical therapy for 12 sessions or 45 days (whichever one comes first) under the Direct Access Act.
